10/06/2022 - Pilot to support the care of High Intensity Users (HIUs) goes live

The Thames Valley and Surrey (TVS) Care Records programme is working hard to share patient information* with health and care staff to support your care. It means information recorded about your health such as illnesses, hospital admissions and treatments can be accessed by different people who are involved in your care, wherever you are seen in Buckinghamshire, Berkshire West, Frimley, Milton Keynes, Oxfordshire and Surrey.

Together with health and care teams across the region the TVS programme has now developed an online dashboard which will display information about those people who make use of health services frequently - in particular A&E, non-planned admissions, primary care, 111, ambulance and mental health services. These people are sometimes referred to as High Intensity Users (HIU) as they often have complex health needs which require support from multiple health services.

High Intensity Users (HIU) are those people who make use of health services frequently - in particular A&E, non-elective admissions, primary care, 111, ambulance and mental health services.

In order to successfully support HIUs with all of their needs in a holistic manner and find out the underlying reasons behind their frequent trips to health services, we need to know who they are. But, because HIUs often use a wide range of health services, in different locations, it can be a difficult and lengthy process to identify them.

24/04/2020 New Senior Responsible Officer appointed to the Thames Valley & Surrey Care Records Partnership

A new Senior Responsible Officer (SRO) has been appointed to the Thames Valley & Surrey (TVS) Care Records Partnership. Jane Hogg will be taking on the role of SRO for the TVS Partnership. Jane is currently the Integration and Transformation Director for Frimley Integrated Care System (ICS) and will be leading the work of the partnership alongside her Frimley ICS role.

02/04/2020 COVID-19 and Thames Valley & Surrey Care Records programme Update

The Thames Valley and Surrey (TVS) Care Records Partnership Board met on 25 March 2020 and discussed how the Partnership can work together in a different way to support health and care professionals in caring for patients in the context of COVID-19.

Partners recognise the importance of being able to share data across the TVS area and the growing importance of ‘anywhere access’ to data in the light of the current crisis.

06/11/19 - 'Accelerate, Enable, Disrupt': Event in partnership with Digital Health

Stakeholders from across the geography came together on 5 November in Maidenhead for the Thames Valley and Surrey (TVS) Care Records Partnership and Digital Health Networks event. The event was hosted by Julie Fisher, SRO for TVS Care Records Partnership and was attended by almost 100 delegates. These included care professionals, CIOs, CCIOs, service managers, and lay representatives who joined us in discussions, debate and heard from experts in the fields of health and digital development.
